Integrated Care for Psoriasis

As a core joint working initiative between the NHS and a partner agency, LEO Pharma UK has funded an innovative programme in psoriasis management. The Integrated Care for Psoriasis programme involves multi-disciplinary healthcare services and professional support to provide holistic care for people living with psoriasis, and ultimately improve treatment outcomes.

Integrated Care Clinics 

Clinics are set up around the UK in partnership with local GP practices, to enable primary care patients to be seen by specialists.

These clinics will:

• Work with GPs to contact patients and introduce the service and its benefits*
• Encourage patients to meet others with the same condition, offering group support and understanding
• Help manage treatment expectations
• Improve patient-healthcare professional relationships
• Build motivation around continued treatment 
• Introduce patients to the support programme
• Improve treatment outcomes
